CDHH's Assistive Listening Device Subsidy Program

With CDHH's direction, United Cerebral Palsy operates seven demonstration and loan centers around the state:

Assistive Device Subsidy Program

A small grant has provided a limited amount of funding for a pilot program to assist individuals to purchase assistive devices needed because they are deaf or hard of hearing.

This program is designed to help people who do not have any other financial resources to help them purchase equipment. The subsidy program will pay one-half (50%) of the cost of the equipment. Eligibility will depend on the cost of the equipment and the resources of the individual. The individual must pay for the other half of the cost at the time it is ordered. Please be prepared to provide proof of being an Idaho resident when applying for the grant.

How to Apply for a Subsidy

If you need help buying an assistive device, you need to contact one of the demonstration and loan centers listed above and request an application. 

The application has a list of the equipment that is available on the subsidy program. Not all types of devices nor all models are available. The staff at the center where you may pick up your application are there to help you through the process of completing your form and choosing your assistive device. 

Step 1: Select equipment from the Subsidy Grant application (see 2008 catalog for details on the equipment).

Step 2: Complete application and turn in to the Demonstration and Loan center.

Step 3: Authorization form is issued to purchase equipment.

Step 4: Form is returned; Equipment is ordered and shipped to your home.
